Family-Physician Program

With the family-physician program “Family-Physicians Centered Care” (HZV), the health insurance companies and participating family-physicians want to work together on a legal basis to improve the quality and cost-effectiveness of health care. The aim is to guarantee medical care of a particularly high quality across the board and to strengthen the central control and coordination function of the family-physicians. The family-physician program is particularly interesting for you as a patient if you need medical treatment more often. In order to be able to ensure optimal care, it makes sense to exchange findings between the physicians treating you and other therapists. With your consent to participate in the family-physician program, you expressly consent to information about your participation as well as findings and therapy recommendations being exchanged and discussed between the physicians and therapists treating you. In individual cases, you can of course control which data being transmitted to your physician. You choose your family physician for at least one year. This family physician is your first point of contact for all medical questions. Specialists may only be called upon after a referral from the family physician. Exceptions: gynecologists, ophthalmologists and paediatricians and of course all medical emergency services. In case of a representation(vacation or illness of your family physician), please visit the representive physician appointed by your family physician. Simultaneous participation in another SVLFG-LKK family-physicianprogram is not possible. Your participation in this family physician program is voluntary and free of charge.

• Quality-assured medical care by your family physician who fulfills special training obligations and maintains certain technical standards
• Treatment according to medical guidelines based on current scientific standards
• Preventive health care (check up) every 2 years instead of every 3 years.
• Coordination and information for the entire treatment process among all physicians and therapists involved by your family physician
• A good exchange of information optimizes your care, e.g. by collecting, documenting and transmitting all available findings relevant for diagnosis and therapy within the framework of referrals to the specialist and before and after inpatient admissions and with aids
• Additionally: Special appointments for working people by arrangement
• Limitation of the waiting time to a maximum of 30 minutes if possible with prior registration
• Recall by the family physician as a reminder for check-up appointments, vaccinations, etc.
